Ladybarn Group Practice - Information for new patients wishing to join the doctors surgery

In order to register as a patient at Ladybarn Group Practice, you will need to live in the practice catchment area. This can be confirmed by ringing the surgery on 08443 87 88 88. The receptionist will ask you a few questions before being able to confirm your eligibility to become a patient at Ladybarn Group Practice.
If you are in our catchment area, you will be asked to come in to the surgery between 8am - 6pm, Mon, Tues Thur or Fri to register. Please bring your medical card with you so we can put you on our system. You will also be asked to fill in a registration form.
If you do not have your medical card, then you will need to find out your NHS number before you come in.
Patients who attend Ladybarn Group Practice special clinics and who are registered with a GP elsewhere, will not be eligible to register with us.
